About the Role – Service Coordinator

As a vital member of our 24/7 Scheduling Coordination Team, you’ll play a key role in connecting clients—many of whom are older Australians—with the compassionate, professional care they need. You will support both aged care and disability clients, helping them remain independent and supported in their homes and communities.

This role is fast-paced, people-focused, and integral to our operations. You’ll be the communication link between our valued clients and our dedicated support workers.

What You’ll Be Responsible For:
  • Coordinating the delivery of in-home and community-based aged care and support services

  • Building and maintaining strong working relationships with our support workers and clients, including older adults and people with varying care needs

  • Ensuring documentation is up-to-date, accurate, and confidential—especially client preferences and care notes

  • Responding to issues or concerns related to service delivery and escalating as appropriate

  • Contributing to continuous improvement by identifying opportunities to better support clients

  • Performing administrative tasks and data entry as needed

We’re Looking For Someone Who Has:
  • Experience in rostering, scheduling or coordination—ideally in aged care, disability, or community services

  • A strong understanding of aged care regulations, funding (e.g., My Aged Care), and care standards

  • Excellent communication and problem-solving skills

  • Confidence working with a diverse range of clients and team members

  • Ability to work both independently and collaboratively in a busy environment

  • Proficiency in time management and attention to detail

  • A valid Driver’s Licence and access to a reliable vehicle

  • A current DHS Working with Children Check and NDIS Worker Screening Check

Location:

This role is primarily based in our Broadview office, with some travel to meet with clients in the community.
