Your Responsibilities

• Deliver person-centred nursing care in clients’ homes in accordance with individual care plans
• Conduct clinical assessments, reviews, and care plan updates
• Provide wound care, medication management, and chronic disease support
• Maintain accurate and timely clinical documentation and reports
• Communicate effectively with clients, families, and the broader care team
• Ensure compliance with aged care standards, policies, and procedures
• Promote health, independence, and wellbeing for older adults in the community

What We’re Looking For

• Current AHPRA registration as a Registered Nurse
• Experience in aged care, community nursing, or a similar healthcare setting
• Excellent clinical assessment, planning, and documentation skills
• Strong interpersonal and communication skills
• Ability to work independently and manage time effectively
• Current driver’s licence and access to a reliable, insured vehicle
• National Police Check, First Aid Certificate, and Manual Handling Certificate
• Experience with wound care and chronic disease management (highly regarded)
